How to write a good business reciept

I need examples of good, clear receipts for money given in transactions; where can I find that?

A reciept should clearly state:

The date
The names of the parties.
The amount of the transaction.
Specifically what was exchanged in return for the money.
Any terms attached like return policies and warranties.
If the transaction is between two private individuals, the signature of the person accepting the money.
