If you have another administrator account, you could revise the lost pw in safe mode.
if not, I am sure you need some tools to remove the lost pw. such as password unlocker tool, you could search it in the google engine.hope it could kindly help you.
The following youtube video could help you better understanding it.

Forgot Windows Vista password?
There are some solutions for you:
Method 1: Remove Windows Vista password through Control Panel
1.Click on 'Start', 'Control Panel'.
2. Click 'User Accounts and Family Safety', and then click the 'User Accounts' link.
3. Click the 'Remove your password' link in the User Accounts screen.
4.Enter your current password in the text box on the next screen, and then click 'Remove Password' button.

Method 2: password reset disk
Windows Vista allows you to create a password reset disk with a USB flash drive in few seconds. It can help you recover Windows Vista password when you forget it. Just try to recover the password in the step showed as follows if you have a password reset disk at hand.

Method 3: a Windows password recovery tool

Step 1. Download and install Windows Password Unlocker
Step 2. Create a password reset disk with the software.
Step 3. Boot your locked computer form the password reset disk.
Step 4. Reset the password with the password reset disk.
