How do i obtain my work history over the last 15 years?

i have worked at a truck-stop, 3 banks, three retailers, a warehouse for staples, krogers food market, and a major retailer til i had a heart attack in august.

There are only two people who have any information on this. You, and your previous employers. What type of information do you need from those anyways? If it's something you don't have, then you must contact your previous employers, and if they don't have it, then it's probably in a land fill somewhere.
